There may be additional costs for diagnostic testing. Function Health offers access to add-on tests not covered by the basic membership fee. There is no coordination between John Hancock and any health benefits you may receive from an insurance policy, health plan, or any other wellness programs you may be enrolled in.
Access to special pricing is only applicable to the first-year membership fee. At the end of the first year of membership, fees for a renewal of your membership will automatically be charged to the credit card on file with Function Health unless the member cancels the membership directly with Function.
John Hancock does not receive individual results from Function Health. For purposes of research, validation of assumptions and evaluation of future health and wellness offerings, John Hancock may receive aggregated and deidentified data from Function Health from time to time.
Like other medical tests you elect, a decision to take one or more lab tests made available through Function Health, and the results of those lab tests, may need to be disclosed in an application for insurance coverage, unless prohibited by law, and thus could affect your access to future insurance coverage and the price of such coverage.
John Hancock is not an affiliate of Function Health. Services related to the lab tests are administered by Function Health and its third-party providers. John Hancock does not provide medical advice, is not involved in the services provided by Function Health and is not responsible for the accuracy or performance of such services.
Lab visits through Function Health are not currently available in Hawaii, Rhode Island, and Puerto Rico. In New York and New Jersey, Quest Diagnostics requires an additional estimated $500 annual lab test charge payable directly to the laboratory at time of service.
